Out-of-Network Insurance Reimbursement:
While Revitalize Mental Health PLLC operates on a private pay basis and does not directly accept insurance, you may be eligible for out-of-network reimbursement from your insurance provider. - Payment at the Time of Service: Clients are responsible for remitting payment for each session at the time of service.
- Receipt for Reimbursement: Upon request, you will receive a detailed receipt, known as a superbill, which includes all the necessary information your insurance company requires for reimbursement.
- Submit to Your Insurance: You can submit this superbill to your insurance provider for potential reimbursement. The amount reimbursed will depend on your specific insurance plan and its out-of-network benefits.
